we are given

[tex] f(x)=x^2+6x+15 [/tex]

we can complete square

[tex] f(x)=x^2+6x+9+6 [/tex]

[tex] f(x)=x^2+6x+3^2+6 [/tex]

[tex] f(x)=(x+3)^2+6 [/tex]

so, graphing form is

[tex] f(x)=(x+3)^2+6 [/tex]..............Answer
